Welcome New Patients

Your intake paperwork will be emailed to you via the patient portal.  Please complete the required patient paperwork prior to your first visit with Dr. Alfano.  You can scan and upload it into your patient portal or fax it to (805) 765-9557.  

 

Insurance Cards

• Please bring your insurance card, photo identification and any copayments to your visit.

  • Dr. Alfano currently accepts the following insurances:  Medicare, Sansum (referral basis only), Blue Shield, Anthem Blue Cross.  *Subject to change.

Other

• Please remember to get a good night's sleep, eat breakfast, bring your glasses, wear your hearing aids (with good batteries), and continue to take your prescribed medications as usual.  If you experience bladder control issues it is most important you wear the necessary undergarments to the appointment.
